Ankle length is best as higher lengths tend to get in the way. We
use ankle length. Besure to get them on the small side. We prefer one size
smaller than normal as then your foot doesn't slide and some of your weight
will be on the sides of the boot. You will have some pain initially but
after a while your feet get trained. My boots have the arch increased so
that the toes are about 2 1/2 inches back toward(touching) the heel. That
increased arch took about four more months to achieve. In all it took about
a year to be real comfortable in walking in the current boots. Much time
was needed with a cane for a balance. The secret is to have them very tight
to provide as much support from the boot as possible.
You really have to practice to stand up straigth. It can be done. I can
wear mine all day and even out in public with the oxfords attached.
